چکیده

The term "elderly" in the cardiac surgical experience refers to patients chronologically aged 2:70 years. Chronologic age is less important than physiologic age; in the physiologically aged patient, techniques used in most surgical procedures are likely to be attended by a markedly increased risk of morbidity and mortality (I). There are four aspects involved in a decision to perform a cardiac surgical procedure in an elderly patient. The first is the preoperative evaluation; the second, the surgical procedure; the third, the postoperative management in the hospital; and the fourth, long-term benefit to the individual elderly patient.

[Evaluating cardiac risk in noncardiac surgery].

In the United States, more than 33 million patients undergo noncardiac surgery each year, at a cost of 450 000 million dollars per year. Approximately 4% of these patients present perioperative complications, at an additional cost of 25 000 million dollars per year.1 The number of cardiovascular complications arising from non-cardiac surgery exceeds 1 million patients worldwide.2 Due to the inc...

Randomized comparison of sevoflurane versus propofol-remifentanil on the cardioprotective effects in elderly patients with coronary heart disease

BACKGROUND It is skeptical about cardioprotective property of sevoflurane in patients undergoing noncardiac surgery, especially in the elderly patients with coronary heart disease. We hypothesized that long duration of sevoflurane inhalation in noncardiac surgery could ameliorate myocardial damage in such patients. METHODS This was a randomized, prospective study. One hundred twenty-one elder...

Right ventricular Hemodynamic Alteration after Pulmonary Valve Replacement in Children with Congenital Heart Disease

Introduction:  In patients who underwent surgery to repair Tetralogy of Fallot, right ventricular dilation from pulmonary regurgitation may be result in right ventricular failure, arrhythmias and cardiac arrest. Hence, pulmonary valve replacement may be necessary to reduce right ventricular volume overload. The aim of present study was to assess the effects of pulmonary valve replacement on rig...

Non-thyroidal illness syndrome and cardiopulmonary bypass in children with congenital heart disease

Abstract Background: The thyroid hormones influence on all metabolic pathways. After heart surgery using cardiopulmonary bypass (CPB), serum T3 decreases and remains low for at least 24 hours. Several studies on pediatric have reported reduction of thyroid hormones after heart surgery. This study aimed to investigate the status of thyroid function tests in children with CPB surgery. Metho...
